How to Pre-Register for a COVID-19 Vaccine

Pre-registration is required for vaccinations at the Mass Vaccination Sites supported by the Commonwealth of Massachusetts. Here are the steps to pre-register online:

1. On your computer or smart phone go to “Mass.gov”
2. Click on the yellow “COVID-19” box on the upper right side. This website has lots of information on many subjects dealing with the Pandemic. Please check this area out for regulations and guidance from Governor Baker and DPH.
3. About a third of the page down you’ll see “COVID Vaccine”; underneath that is a gray box “Pre-register for COVID-19 Appointment”. Click on that.
4. This section will have many answers to questions you may have regarding vaccinations and mass vaccination sites. Halfway down is “How to Pre-Register”. This site will give you information on what will be asked so that you may obtain that info prior to filling out the application. Now click on the gray box “Pre-Register for a Mass Vaccination Location”.
5. Read this page then click “Next”.
6. Fill out all fields, especially the ones with an asterix. If you are filling this form out for a friend or relative check the appropriate boxes.

The two closest vaccination sites for Fairhaven are Gillette Stadium in Foxboro and the former Circuit City in Dartmouth. If you don’t have access to a computer or are unfamiliar with website navigation, call “2-1-1” and speak to a live person who will assist you in registering for a vaccination at one of these Mass Vaccination Sites.

• If you are home-bound and would like to register for an in-home vaccination shot, call 1-833-983-0485. They offer in-home service Monday to Friday, 9:00 am to 5:00 pm.

As a reminder, free COVID-19 testing is available on Saturdays from 10:00 am to 12:00 noon at the Fairhaven Emergency Management Building located at 150 Sconticut Neck Rd. by the Fairhaven Fire Department, no appointment is necessary. This program is dependent on funding which may be withdrawn without notice so get tested soon.

As always, remember to wear a face covering in public, avoid large gatherings and maintain a minimum of 6’ of social distancing. Wash your hands often or use hand sanitizer.
